Clicking on the link below will take you to the login page. You
will be asked to enter
your user name and password. If you do not remember your password,
just click on the
appropriate link, and it will be sent to the email address you
registered with.
Once
you have logged in, you will be taken to the new online version of the
WPIF. You may
make any changes you want, and take as long as you wish to complete it.
If you choose to
leave the website before you complete it, you will be asked if you wish
to save the changes you
made. That will ensure that you do not have to continue to make
the same changes more than
once. When you are finished, click the "submit" button and it will
forward your form to us
immediately.
Once
your WPIF has been received, a confirmation will be mailed to you in a
timely manner.
